Does anyone have a patch that would allow the asterisk port to be installed
without (lib)curl?

FWIW asterisk (1.4) runs fine without curl despite startup warnings
(assuming you have no curl rules). Curl is required by portinstall but
can be pkg_delete'd post-installation.

Upgrading to the 1.6 port provides an option to exclude curl at
compile-time ("CONFIGURE_ARGS+= --without-curl" via `make config`).
