www.pikatech.com is about to release an embedded Appliance for Asterisk. The unit has optional BRI, FXO and FXS interfaces. It's entirely open: we ship it with Asterisk 1.4 and selected other publicly available tools but the intent is that software developers put their own software on it.
It has a customer friendly LCS screen (2*20 char), USB, ethernet, an SD card holder, a standard FXS port to connect a faxmachine to, and audio In/Out jacks.
Pricing: public listprice of a unit with a BRI module is below 500 euro's.
