Just in case anyone else is having this issue, I'm reviewing my trixbox
setup and I think all you really need to do is add "#include
pizza_demo.conf" to the extensions_custom.conf file, and then set up a
route in FreePBX (either as an inbound route or via the IVR) to the
custom app called custom-pizza,s,1.
