i've noticed that the latest version of the wctdm driver (still wcfxs in
freebsd) has a few new PCI device ids supported. your card may be one of
the new ones.

that looks like a Rev E/F card. well, i really can't tell why the Modules
are not being installed, other than to say that i've got the wcfxs driver
working with a TDM40B quite ok, with the exception that modules without a
phone plugged in will eventually power alarm themselves out of order. only
a module reload seems to fix that.
